The HAS-BLED score is a simple risk stratification tool used to estimate the 1-year risk of major bleeding in patients receiving anticoagulation, particularly for Atrial Fibrillation (AF).
Risk Stratification (Annual Bleeding Rate):
Score 0-2 (Low-Moderate Risk)
Annual major bleeding risk is generally low (1.0% to 2.0%). Benefits of anticoagulation usually outweigh the risk.
Score 3 (High Risk)
Annual major bleeding risk is elevated (around 3.7%). This warrants *caution* and *close follow-up*, but is NOT an absolute contraindication to anticoagulation.
Score 4+ (Very High Risk)
Annual major bleeding risk exceeds 5%. The need for anticoagulation must be carefully weighed against the risk. This requires intense modification of modifiable risk factors.
Modifiable Risk Factors:
A key feature of HAS-BLED is identifying reversible risk factors that can be managed to lower the score. These include:
- Uncontrolled Hypertension (H)
- Concomitant use of NSAIDs or Antiplatelets (D)
- Excessive Alcohol Consumption (D)
- Labile INR (L) – Better management of Warfarin therapy.
Note: The maximum possible score is 9, as Abnormal Renal/Hepatic function contributes 2 points in the original definition.
