Clinical Disclaimer: The Neck Disability Index (NDI) is a standardized clinical assessment tool widely utilized in physical therapy, chiropractic, and orthopedic practices across the United States. This online calculator is provided purely for educational and self-assessment purposes. It is not intended to diagnose medical conditions or replace formal medical evaluation. If you are experiencing persistent neck pain, numbness, tingling, or weakness, please consult a licensed healthcare professional or physical therapist.
Understanding Your Results
Once you complete the assessment, your percentage score places you into one of five functional disability categories:
| NDI Score (%) | Raw Score (out of 50) | Disability Category | Clinical Interpretation |
| 0% – 8% | 0 – 4 | No Disability | Mild discomfort may be present, but daily function and activities of daily living (ADLs) remain unaffected. |
| 10% – 28% | 5 – 14 | Mild Disability | You can manage most daily activities independently, though certain tasks may cause mild strain or require extra effort. |
| 30% – 48% | 15 – 24 | Moderate Disability | Pain significantly interferes with daily activities such as driving, reading, working, or sleeping. Physical therapy is often recommended. |
| 50% – 68% | 25 – 34 | Severe Disability | Neck symptoms pervade almost all areas of daily life. Work and recreational tasks are restricted. Medical evaluation is advisable. |
| 70% – 100% | 35 – 50 | Complete / Severe Disability | Severe functional limitation or bed-bound state. Immediate consultation with a healthcare provider or spine specialist is strongly encouraged. |
